چکیده

Scaphoid nonunion is a common but difficult problem for hand surgeons. The diagnosis of scaphoid nonunion is often delayed, and therefore, treatment must be tailored to the type of fracture, the duration of nonunion, and the presence or absence of resulting arthritis. This article reviews the diagnosis and work-up of scaphoid nonunion, classification schemes for scaphoid nonunion, and various treatment options, including internal fixation, nonvascularized and vascularized bone grafting, and salvage procedures. (Plast. Reconstr. Surg. 112: 1666, 2003.)

Scaphoid Nonunion.

The natural history and treatment of scaphoid fractures and subsequent nonunions have occupied a substantial portion of the orthopaedic literature. The authors examine the role of modern diagnostic tools in making an earlier diagnosis of scaphoid nonunion, in more accurately determining the displacement and angulation of the fragments, and in identifying the presence of avascular necrosis. They...

Scaphoid fractures and nonunions: diagnosis and treatment

BACKGROUND Scaphoid fractures are commonly seen in orthopedic practice. An organized and thoughtful approach to diagnosis and treatment can facilitate good outcomes. However, despite optimal treatment, complications may ensue. In the setting of nonunion or an avascular proximal pole, vascularized bone grafting may be needed. METHODS AND RESULTS In this article we review the literature regardi...

[Results of treatment of scaphoid nonunion with microvascularized bone grafts of the 1, 2 intercompartmental supraretinacular artery and osteosynthesis].

OBJECTIVE To analyze the results of treatment of scaphoid nonunion with questionable viability of the proximal fragment, but without significant deformity or collapse, using a vascularized bone graft of 1,2 intercompartmental supraretinacular artery (1.2 ICSRA) and subsequent osteosynthesis. MATERIAL AND METHOD A retrospective study was performed on 10 male patients with a radiographic diagno...

Scaphoid nonunion: correction of deformity with bone graft and internal fixation

The scaphoid is the most commonly fractured bone in the carpus. Although greater than 90% of scaphoid fractures unite with cast immobilization, failure to heal remains a clinical reality, particularly when the fracture is displaced or associated with intracarpal instability [1–3]. Dabezies [4] reported a 55% incidence of nonunion and a 50% rate of proximal pole avascular necrosis (AVN) in scaph...

Treatment of scaphoid fractures and nonunions.

Scaphoid fractures are common but present unique challenges because of the particular geometry of the fractures and the tenuous vascular pattern of the scaphoid. Delays in diagnosis and inadequate treatment for acute scaphoid fractures can lead to nonunions and subsequent degenerative wrist arthritis. Improvements in diagnosis, surgical treatment, and implant materials have encouraged a trend t...
